📌 Remember! If you are already a patient in any Gorebridge area surgery, you can't register with us. This applies to Newbattle or Pathhead surgeries' patients.

2️⃣ Step 2: Fill out the New Patient Questionnaire 📝

3️⃣ Step 3: Fill out the Application to Register with a General Medical Practitioner - GPR (Scotland) 📝

4️⃣ Step 4: Visit our reception team  

Come by after 10.30 am from Tuesday to Friday within 14 days of completing the forms. If you don't, you'll need to start the process again. Don't forget to bring your valid ID! 🆔 

What's next?

Once the practice approves, you'll be officially a part of our list! We'll transfer your medical records to our practice, which can take a few weeks📁.  You may also get an invite for a new patient check-up/assessment🩺. 

📎 Please bring two forms of identification.

 Acceptable ID includes:

- Passport 🛂

- Drivers Licence 🚗

- Official ID card from Public Services body 🏛️

- Student matriculation card (current year) 🎓

- Recent utility bill (within last 3 months) 🧾

- Council Tax document 🏡

- Television Licence 📺
