PRACTICE LIST

 

 

With effect from the 6th February 2017, until further notice, our practice list is Restricted for new patients.

 

This has come about as a result of a combination of factors including an increased list size and increased workload. We have come to the decision to restrict our practice list in order to provide safe, quality and accessible care to our existing patients.

 

Currently, five out of twelve General Practices across Midlothian have restricted their practice list due to shortage of doctors and rising demand.

 

We will be operating on a “restricted, but open” basis; this means that for the time being we will only register certain categories of patients as follows:

 

  • New babies of existing patients
  • Immediate family members of existing patients who become resident within the registered patients address
  • Temporary resident 
  • Assignments/ allocations   If you fail to find a practice where you can register, you will be required to call practitioner services on the number below where they will allocate you to a practice by letter. This is to ensure a fair allocation of new patients across all practices in the area.Please bring the letter from the Health Board with you when you attend your allocated practice for the registration forms

 

NHS Lothian has requested that patients should to seek registration with a practice that covers their address. This may mean approaching more than one practice, including practices declaring a restricted list. Whilst NHS Lothian would like all patients to be able to register with their nearest practice this may not be possible or may take some time until the list turnover allows them to register.

How to Register with the practice

register_nowIf you fall in to one of the categories above you will be asked to complete a registration form. It is also helpful if you can provide your Health Board medical card. In addition to a completed registration form, you will be asked to provide 2 separate forms of identification, one of which must provide proof of address.

All newly registered patients will be invited to attend the practice for a new patient check-up/assessment which will usually be conducted by our nursing staff.

It is preferable if you register with the practice as soon as you move to the area rather than waiting until you are ill. This means that we can try and get your medical records before you actually attend the surgery.

You may wish to see a particular doctor and if so, the reception staff will try to give an appointment with your chosen doctor, however please bear in mind that due to workloads and annual leave this may not be possible.

Please note that new patient registrations will not be processed between 8.30am - 10.30am, and all day Monday.

 

Online 'pre-registration' with the practice

When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Please note that new patient registrations will not be processed between 8.30am - 10.30am, and all day Monday.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. The online version of this file is below and must be completed and submitted at the same time.  When you come to the surgery you will be asked to sign this form to confirm that the details are correct. Please ensure you complete both the pre-registration form and the online medical questionnaire in full.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
